15. Please provide your medical history (i.e.: do you or have you had any other diseases? Cancer, Lyme, Thyroid, AIDS, other?)- have heart failure,auto-immune disease,thyroid. Was very sickley child with asthma and many allergies. Believe I contracted Lyme disease about 25 years ago.
