5. Do you live in a heavily treed area? Never have.

9. Your occupation at onset? Student, Social Worker, Customer Service Rep

11. Have/had you traveled to California, Florida or Texas? Europe? Other? Cali.
12. Do you/have you garden, hike, camp or spend allot of time outdoors? Nope.
13. List your activities and hobbies. Indoors stuff. Sedentary.
14. Have you used a public whirlpool or pool? No.
15. Please provide your medical history (i.e.: do you or have you had any other diseases? Cancer, Lyme, Thyroid, AIDS, other?). PCOS, asthma, allergies, arthritis, weak joints, all before. Now, same, just much worse.
16. Do you smoke? Yes
17. Any medications you have taken or are taking? no.
18. Do you drink more than 8 cups of coffee a day? never drank coffee
19. As far as lesions, have you had more than 10 at a given time? never had any.
20. What part of your body did the lesions start? none
21. Is there any area that they are most prevalent? If so, where? n/a
22. Is one side of your body/face more affected than the other (i.e.: left vs. right)? no
23. Please list your current symptoms (brain fog? vision disturbances? fatigue? nausea? weight loss/gain etc?) vision and hearing degradation, fatigue, memory loss, spontaneous paper cut-like scratches that end in tiny puncture wounds that ooze and sometimes a little spike will come out of them for a bit and then retreat, small crystal like shards --green and white--coming out of skin, worms, insects, cocoons, white goo, black and white specks coming out of skin. lymph node swelling and pain. intestinal and general internal pain.
